7th & 8th Grade Physicals for Spring School Sanctioned Soccer

GMSD physical day will take place on June 28th at the high school.

Please enter through the MAIN entrance.

The cost is $15.00 per athlete - time schedules per grade are listed below.

If you are not able to make your scheduled time, please show up within one of the other time frames.

PIAA Physical packets (Updated July 17th, 2024) and Athletic Trainer forms can be found by following the link - https://www.generalmclane.net/athle.../physical-information/

Please fill out Sections 1 - 5 before arriving if possible. Copies will be available in the high school office.

**All physicals and athletic trainer forms must but turned into Courtney Behnke at the high school main office!! Please do not turn any paperwork into coaches.
