Village Vitality: A Community Wellness Day

Attendees will enjoy interactive workshops, wellness demos, and access to a variety of wellness resources, all while building community and receiving tools for healthy living.

IMPORTANT NOTICE: By Registering For This Event You "AGREE" To The Following Waiver, Release, and Indemnity Agreement

DISCLAIMER. By acknowledging the below, I hereby register my son/daughter/ward/or myself for the One Village Alliance "Community Easter Egg Hunt" Event on 18 April 2025, subject to the following terms and conditions established in this Waiver, Release, and Indemnity Agreement. It is understood and agreed that completion of registration constitutes a legal and binding agreement between One Village Alliance (including collaborating/partnering organizations, individuals, volunteers, students, and other affiliates), and the undersigned.

PHYSICAL DAMAGE. I specifically understand and agree to be responsible for all damage to One Village Alliance property, or the property, that occurs as a result of my, my child or my ward's fault or negligence. I will immediately report any such damage to a member of the event staff.

RELEASED PARTIES. I specifically understand, agree to and do hereby release One Village Alliance, their affiliates, administrators, staff, employees, contractors, volunteers, and collaborating partners (the “Released Parties”) from any and all obligations, claims or financial liability resulting from any incidents, mishaps or accidents involving me, my child or ward while participating in or observing any program activity including, but not limited to, athletic, recreational or flying activity, or while in transit to or from any of these activities.

PARTICIPANT INJURY. I specifically understand, agree to and do hereby release the Released Parties from any liability and costs related to physical ailments, accidents, or death occurring to my child or ward incurred while he or she is enrolled in the "Community Easter Egg Hunt." This includes, but is not limited to ailments, accidents, or death as a result from actions deemed reasonably appropriate by One Village Alliance, and other collaborating partner staff in the event of an emergency. I assume the risk of any financial or other loss which may be incurred in the event my child or ward withdraws or is expelled from the “Community Easter Egg Hunt ” event for any reason.

PHOTO/PROMOTIONAL RELEASE. I specifically understand, agree to and hereby consent to the use in promotional materials, videos, or any photograph in which me, my child or ward may appear. By completion of the registration, I understand that the foregoing terms and conditions will be legally binding on me (individually and as guardian for my child or ward), my heirs and assigns.
