Paperwork Dropoff

‼️Attention Witches‼️

Below is a list of the MANDATORY paperwork that is required for cheerleading and football.

🏈 Football Families: all players will be required to turn in current physical or medical clearance form. New Football players must also turn in a copy of their birth certificate.

📣 Cheerleading Families: all paperwork besides your child’s birth certificate, physical/ medical clearance form and report card will be given to you in your child’s registration folder. I will have these folders at our 2025 Parent Meeting on Thursday, June 26th at 6pm at the Jean Levesque Senior Center or at any of the paperwork turn in nights. These folders must be complete NO LATER than July 31st!!!! Your child will NOT be able to start practices until ALL paperwork is complete.
