AGREEMENT TO THE FOLLOWING WAIVER IS REQUIRED FOR EVENT PARATICIPATION

WAIVER AND RELEASE

I, the undersigned participant (or parent/legal guardian if under 18), understand and acknowledge that participation in the Eritrean Youth Mentorship Program (EYMP) Soccer Tournament involves inherent risks of physical injury, illness, and other hazards associated with competitive sports.

By signing this form:

1. I voluntarily assume all risks related to participation, including but not limited to collisions, contact with other players, weather conditions, field conditions, or equipment failure.
2. I hereby release and hold harmless the tournament organizers, referees, volunteers, sponsors, facility owners, and affiliated parties from any liability, claims, or demands related to injury, illness (including COVID-19), or property damage.
3. I certify that I (or my child) am physically fit to participate and have not been advised otherwise by a qualified medical professional.
4. I understand that I (or my child) am solely responsible for any medical expenses or other costs that may arise from participation.
5. I grant permission for emergency medical treatment in the event of an injury or illness during the tournament.

PHOTO/VIDEO RELEASE: By attending the cup, I authorize the use of photos/videos taken during the event for promotional purposes.

Code of Conduct Policy

You understand that EYMP has a Code of Conduct Policy in place, and you agree to comply with the policy:

We expect the following from each other:

1. Respect the Game. We celebrate competition, but we are a community organization, and having fun is the most important thing. Always treat the game and the officials with respect.
2. Respect the Officials. We work hard to recruit the best referees we can. We may disagree with calls, but we treat referees as fellow humans and show respect and courtesy.
3. Respect the Opponent. It’s just a game! Don't taunt, mock, bait or otherwise put down your opponent. Foul, abusive, or racially motivated language has no place in our tournament.
4. Respect Yourself. Be the teammate you want to have; integrity is an expectation. Treat everyone on the field as you would a friend or family member. We share the same goal of sports, fun, and community!

Note: Failure to comply will result in being removed from the event.

Weather & Game Reschedule Policy

You understand that EYMP has a Weather Policy in place, and you agree to comply with the policy:

Severe Weather

If severe weather is forecasted, we will be monitoring the situation throughout the day. If Montgomery County closes the fields due to severe weather or player safety is at risk and we must cancel the tournament, all players will receive an email and/or text notification as soon as the decision is made.

Reschedule

If the tournament isn't played due to severe weather, attempts will be made to reschedule for a later date.

Refund Policy

You understand that EYMP has a Refund Policy in place, and you agree to comply with the policy:

1. All registration fees are non-refundable. Registering for the tournament is a commitment to participate in the event.
2. In case the event is cancelled due to severe weather, and we are unable to reschedule the event, a refund will be issued.
