Discharge planning plays a vital role in the health outcomes of patients during and after an acute stay and/or placement in a care facility. This course is intended to build upon the competence and confidence among social work, health, and other professionals, in their ability to engage in a discharge planning process that is effective, collaborative, and sustainable.
