Advanced Care Planning

This workshop requires attendance both Saturdays:
* On August 2nd the Workshop will focus on the ins and outs of Advanced Care Planning by experienced experts provided by Hospice Austin. Every participant will receive a packet of information and legal forms.
* On August 9th the Workshop will cover any unanswered questions, discussion and additional information. Witnesses and Notaries will be available with the goal of completing and finalizing legal documents such as Medical Power of Attorney.
