Are you interested in enrolling your child into Head Start Preschool at Porter-Leath?
Join us Wednesday July 30th and Thursday July 31st 9 a.m.-1 p.m.
To be eligible for enrollment in Head Start Preschool:
- Family Lives in Shelby County
- Head Start Preschool students must be 3 or 4 years old on August 15, 2025
- Priority is given to students who meet Federal Poverty Guidelines and who are categorically eligible: (SNAP, TANF, child receives SSI or if child is in foster care.)
- Please bring the following with you to the enrollment event:
- Parent’s Photo Identification
- Proof of Parent’s Address (i.e. MLGW bill, lease or mortgage in parent’s name)
- Parent’s Proof of Income (3 most recent check stubs, 2024 W-2 or Tax Return, Proof of Families First, SNAP, SSI Disability or Child Support) PLUS Proof of Income for all Members of the Family
- Certified Copy of Child’s Birth Certificate
- Copy of both Child's Social Security Card and Insurance Card
- Certificate of Immunization with Current Physical Exam (within 12 mos.)
- Well Child Summary
- Proof of child's dental exam, if available
