“Health Care Directives, explained”

An opportunity to create and complete your own Health Care Directive by Dr. David Lakes, and Marian Cremin, LCSW, Medical Social Worker, DipIBLM”

An advance healthcare directive is a legal document that outlines your preferences for medical treatment in the event you become unable to make your own decisions due to illness or injury. It essentially communicates your wishes to your healthcare providers, family, and friends, ensuring your preferences are honored when you can't express them yourself.

In this hands-on practical presentation we will distribute directive forms which participants can fill out between the 7/14 and 7/21 sessions. On 7/21 we will any answer questions and witness signing your document as needed.
