Appointments are quickly filling up! If your student is interested in participating in school sports for the 2025-2026 school year, please take advantage of this opportunity.

H.B. Lee Middle School has partnered with Multnomah County Health Department to host a Sports Physical Mobile Clinic at our school on TUESDAY, AUGUST 12th from 11:00AM to 6:00PM.
This clinic is BY APPOINTMENT ONLY.

Please follow the steps under the section *** TO SCHEDULE AN APPOINTMENT***. All necessary forms have been provided on that page. Please complete the forms and bring them with you to your appointment. Space is limited, so if your student would like to participate in sports for the 2025/2026 school year, they will need to have a current sports physical on file in order to do so.
