📌 Important info for physicals:
No need to bring a form — we’ve got you covered! You’ll just fill out the parent section during check-in.
We’ll provide a copy of your child’s immunization records. If you don’t vaccinate, no problem — you’ll just complete the online exemption form.
This visit is not your child’s annual wellness exam. You’ll need to schedule that separately around their birthday.
Physicals will be billed to insurance and may have an out-of-pocket cost after processing. We’ll send you a statement afterward.
💳 Self-pay? It’s $65 for new patients and $55 for established ones.
