PLAQUE ORDERS:

If you would like to purchase a plaque, you can do so through our preferred vendor, Providence Picture Frame. The plaque can only be purchased after the event, and you must make arrangements with the vendor for pick up.

Ticket Information:

If you are a Top Doctor, please select, “Top Doctor, No Plaque” and there will be no fee to attend the event. Enter NOPLAQUE at checkout for there to be no fee.

Please register yourself by Sunday, April 27th. If you’d like to bring additional guests, limited tickets are available for $75, not including Eventbrite fees, and are on a first come, first serve basis. Guest tickets can be purchased on the Eventbrite page during registration. Please note: This is a 21+ event. Tickets are non-refundable.
