Check or money order only will be accepted the day of the program and should be made payable to AMBA Wellness Programs.

Cost = $65.00 for the AMBA Panel which includes coronary disease, kidney disease, anemia, liver disease, diabetes, calcium, choride, potassium, sodium, carbon dioxide.

Optional tests available:
$27 - prostate specific antigen
$13 - TSH
$26 - vitamin D
$13 - hemoglobin A1C
$22 - insure colon rectal kit
