Navigating a hospital stay or care transition can feel overwhelming — but you don’t have to do it alone.

Join us for “From Hospital to Home and Everywhere in Between,” an insightful Lunch & Learn presentation by Cindy Johnson of Everbright Legacy Law.

Cindy, a licensed social worker, will walk you through the discharge planning process, from what happens before you leave the hospital to how to ensure safety, support, and continuity of care once you’re home (or in rehabilitation, long-term care, or another setting).

Whether you’re preparing for your own recovery or supporting a loved one through the process, this session will help you feel informed, empowered, and ready to navigate the next steps with confidence.
