Advance Care Planning 101

Making decisions about the health care that you want as you live the end of your life is complicated. But thinking it through and talking about it now might be some of the most lifegiving things you can do - for yourself, for your loved ones, and for your health care providers. Don't avoid it now and miss your chance to live that time the way you wish you would have.

In this session, we will cover:
Advance Directives (Health Care Proxy, Living Will, and Do Not Resuscitate/DNR Order)
How to Think About & Talk About What Matters Most to Us
Resources to Support You, including Advance Directives: Making Your Wishes Known and Honored and Your Conversation Guide: How to talk about what matters to you and have a say in your health care.
