Living Well With Chronic Conditions

Living Well with Chronic Conditions is a fun-skill-building program designed for persons with chronic diseases. It helps individuals take day-to-day responsibility for the care of their chronic condition. At the end of the series, participants will gain skills necessary to self-manage their condition and work effectively with their health care providers.

Anyone living with a chronic condition or family members, friends, or caregivers of an individual with a chronic condition can take this program.

You do not have to attend all the sessions if you are unable. It is highly recommended to experience the full benefits of the program.
