Humansville Family Medical Center will offer FREE sports physicals for middle and high school athletes on Wednesday, July 30, Thursday, July 31 and Friday, Aug. 8, 8 a.m.-4 p.m.

You do not have to be an established patient

Sports physicals are free on the specified dates and times only. On all other days, sports physicals are $44 (payment due at time of service).

Need a wellness exam too? The sports physical form can be completed during that visit! Most insurance plans cover one wellness exam per calendar year at no cost. Call to schedule!
