A medical crisis can happen at any age, so it’s a good idea to have health care plans for the future. Advance care planning is an important step to ensure you get the medical care you want.

What is Advance Care Planning?

Advance Care Planning is a process of planning for future medical care in case you are unable to make your own decisions.

Advance Care Planning helps you and your loved one have conversations about your values, preferences, and wishes related to health care. If you are unable to speak for yourself due to illness or injury, it guides healthcare decisions and ensures that your personal preference are honored.

Our trained facilitators can help you:

• Initiate conversations

• Provide assistance with advance care planning

• Communicate wishes and any documents with your family, friends and healthcare provider.

• Notarize the Advance Directive

How can an Advanced Directive help you?

If you have an Advance Directive, your wishes will be known and will only used when you cannot speak for yourself. Our Advanced Care Planning sessions are designed to educate you on advance care planning and offer you the option to complete an individualized advance care plan.

Join us at a FREE Drop-In Session

Drop-in sessions are offered every 2nd Wednesday of the month from 10:30 - 11:30 am at the Olivia Hospital & Clinic.

If you have an Advance Directive that you would like to review at the session, please bring a copy with you. Advance Directive forms are available if you are interested in completing one.
