Guiding an Improved Dementia Experience (GUIDE) – A New Dementia Care Management Program

As Maine’s population ages, so does the prevalence of dementia; in 2025, it is estimated that there are 35,000 people aged 65 and older with Alzheimer’s residing in Maine. Navigating the healthcare system and a dementia diagnosis can be overwhelming, but it doesn’t need to be. Join Andwell Health Partners staff as they share the exciting launch of GUIDE (Guiding an Improved Dementia Experience), a dementia care management program (coming July 2025). GUIDE, an 8-year CMS pilot, is focused on improving patients and caregivers quality of life, reducing caregiver strain and burden, and helping patients remain in their homes and communities longer. Staff will dive into program eligibility requirements, referral processes, and GUIDE’s nine care domains.
