Join Brightmoor Hospice as we share the importance of advanced care planning using the “Five Wishes” Program

Five wishes is advance care planning encouraging those around us to have conversations about what good care means. The foundation of the program is the easy-to-understand and use advance directive. This 12-page booklet meets the legal requirements in all 50 states and includes important personal, emotional, and spiritual matters in addition to medical wishes.
