$1 Sports Physical

Cibola County Residents: For just $1, bring your child in for their Sports Physical Exam on Thursday July 31st from 5PM-7PM at Cibola Family Health Center in Grants, NM.

➡️Children 7th-12th grade, please bring a completed physical form signed by a parent or guardian.
➡️ You must have a parent/guardian signature to participate.
NO SIGNATURE --> NO PHYSICAL
➡️ A guardian must be present during the physical.
➡️ Exact Change Preferred.
