Better Choices, Better Health Chronic Disease Self Management Class

Better Choices, Better Health is a FREE interactive 6-week program that helps adults and caregivers manage the symptoms of chronic diseases. Each workshop is led by two volunteer leaders.

If eligible, participants may receive three boxes of fresh produce provided by funding from the Prairie Meadows Betterment Grant.

This program will help you learn:
• How to communicate effectively with family, friends, and health professionals
• Techniques to deal with isolation, frustration, fatigue, and pain
• Healthy eating and exercise tips
• How to evaluate treatment options
• How to create action plans
