Community Health Improvement Plan Session

A community health improvement plan is a long-term, systematic effort to address public health problems in a community while also preventing poor health outcomes.

This process starts with a series of community conversations focused on understanding health needs in our counties. Community coalitions then help prioritize how to improve health by developing programs and events.

We want to hear from individuals who live, work, or play in Washington and Ozaukee County to help identify priority health issues.
