Completed applications can be submitted via email or dropped off at our Home Farm Stand at
3437 East Sauk Trail Chicago Heights, IL.
Spots are not secured until payment has been made and application has been accepted! You will receive a confirmation email that your application & payment have been received, securing your spot. We accept cash, card or checks made out to Smits Farms. If you'd like to pay electronically, please clarify that in your email with the attached application and we will send you a link where you can do so.
