Documentation Required to Apply:
Families must bring the following items to be eligible for
uniform assistance:
Valid Indiana Driver’s License or State-Issued Photo ID
2) Proof of current residence
- If the address on the ID does not match your current address, please bring a recent utility bill or bank statement
3) Original Social Security Cards for all household members
- Photocopies, scanned images, or digital
versions will not be accepted
4) 2025–2026 School Enrollment Verification Letter
- Applicants must reside within Calumet Township
Eligibility Requirements
To qualify, families must meet at least one of the following:
Received assistance from Calumet Township within the past 12 months
2) Current SNAP recipient (approval letter required)
3) Student receives a Choice Scholarship (scholarship award
letter required)
4) Household resides in a designated low-income census tract
