Gardner - Well Child Clinics

CHC is now offering June Well-Child Clinics!
(In addition to our August & September Clinics)

The Well Child Clinics are Walk-in/No Appointments Needed and will be happening at our community health centers.

These clinics are for CHC patients & those who do not have a primary care provider and need to establish care.

Fitchburg Community Health Center, 326 Nichols Road
(June 2nd & June 6th, 1:00pm-4:20pm)

Leominster Community Health & Urgent Care Center, 165 Mill (June 3rd & June 5th 1:00pm-4:20pm)

South Gardner Community Health & Urgent Care Center, 529 Timpany Blvd.
(June 4th & June 11th 1:00pm-4:20pm )

We have informational flyers available in English, Spanish and Portuguese.
