Better Choices, Better Health
Better Choices, Better Health, also known as the Chronic Disease Self-Management Program (CDSMP), helps adults and caregivers manage symptoms of chronic diseases like heart disease, arthritis, diabetes, depression, asthma, and more.

This evidence-based program meets weekly for 2 ½ hours over 6 weeks at community locations, offering mutual support to build confidence in managing health and maintaining active lives.'
