Wilson Middle School Sports Physical & Immunization Clinic

School will soon be starting, which means the start of a new season of athletics at our local schools. All Care will be hosting a sports physical clinic at the All Care Mobile Medical Clinic at Wilson Middle School on August 5th from 8am-12pm.

Athletes from Abraham Lincoln, Wilson Middle School, and surrounding schools can also attend this health event. Please bring your insurance card if available. Medicaid, commercial insurance, & uninsured welcome. No one will be turned away based on their ability to pay. Call (712) 325-1990 to make an appointment.

To participate, student-athletes must first complete their annual sports physical paperwork found at www.allcarehealthcenter.org/wp-content/uploads/2024/02/ACHC-Sports-Physical-Packet_English.pdf.
