The Impact Workshop is designed for the Boots On The Ground, Community Healers and Community Health Leaders who are implementing programs and services that require comprehensive knowledge of where Community Health Work and similar roles such as: Case Management, Healthcare Coordination, Community Resource Navigation and Lead Care Management all intersect.
We will discuss vital topics like data collection and data extraction; measuring outcomes; creating a cohesive CalAIM ecosystem for your enrollees through engagement and collaboration; AND utiliziing compassion as fuel to takes your efforts to new heights and much more.
