Discharging from the Hospital
Join Jane Barker of Shiloh Angels Homecare for this program aimed at making sure you have all the information you need when discharging from the hospital. You will go over the need to know info regarding discharging from the hospital to rehab, rehab to home, what services are available such as VNA, and how to navigate the system when it comes to returning home from a hospital stay to make sure you are getting the correct in-home care you need. You must register in advance.
Registration opens Monday, June 23rd at 2pm.
