In honor of our cherished team member, Yolonda Williams, this outreach aims to carry her work forward in bringing Flagler Cares’ services directly into the community she cared so deeply about.

Services Offered:
- Screening for Behavioral Health & Care Coordination Services
- Help Applying for Food Stamps, Medicaid, & Medicare Savings Plan
- Referral and Connection to Resources

2nd and 4th Wednesday of each month
