Athletic Physicals

📢 Attention Parents and Students:
Physicals will be held THIS week! Please ensure the front of the physical form is completely filled out by a parent before the appointment—this is required.
• Med Now will be here Tuesday, the 12th, from 10 AM to 12 PM.
• Accepted insurances are CareSource, Peachstate, and Amerigroup.
• The cost is $20 if your child does not have one of these insurances.
• Physical forms are available in the office or from Coach Ayers.
