Summit County Birth Certificate and Name Change Clinic

Trained volunteer attorneys will be available to help you complete forms and discuss your questions concerning legal name change, birth certificate correction, and more from 10:00am until 3:00pm.

Notaries will be available! Probate magistrates will also be able to process filings and issue orders on site!

There are costs associated with parking and legal filing fees: approximately $125.00 for name changes, $40.00 for birth record corrections, and $10.00 for parking.

Financial assistance will be available for those who qualify. Applications will be taken on site.

What to bring:
Government-issued photo ID*
Copy of birth certificate
Cash, check, or money order for the filing fee(s)
