- Copy Of Most Recent Shot Record (Ages 5-18)
- Copy Of Most Recent Physical (Ages 5-18)
- Week 1 Payment ($55) (Ages 5-12)
- Membership Fee ($20) (Ages 5-18)
- Registration Fee ($25) (Ages 5-18)
