Advance Care Planning is about making sure your values, goals, and wishes for future health care are known and respected. This workshop provides a gentle introduction to what advance care planning is, why it matters, and how to start the conversation with your whānau and health professionals.

Participants will be guided through practical steps and resources to help you think ahead and feel more prepared for whatever the future may hold.
