NOTICE OF PUBLIC HEARING AND WRITTEN COMMENT
By the requirements of the Illinois Health Facilities Planning Act, notice is given of the receipt of an application for change of ownership of Oak Hill Senior Living & Rehabilitation Center, 623 Hamacher Street, Waterloo, Illinois (#25-031). The Applicants are Monroe County, Illinois, and Accolade Ventures LLC.
A Public Hearing has been scheduled for Wednesday, October 8, 2025
at 4:00 p.m. at the
Monroe County Annex Building
901 Illinois Avenue
Waterloo, Illinois
This project is scheduled for the November 18, 2025, State Board Meeting. The public may also submit written comments on this project. All public written comments must be received by October 29, 2025. HFSRB will post its findings in an HFSRB Staff Report, which will be available via the Internet on November 5, 2025. The public may submit written responses in support of or in opposition to the findings of the HFSRB staff. All written public comments about the HFSRB staff findings must be received before 9:00 am, November 10, 2025. The internet address used to access this report is: www.hfsrb.illinois.gov
