Advance Care Planning Training Day

This training day will be an opportunity to explore the new version 5 of the Children and Young People’s Advance Care Plan and learn how to plan for the future where that future is uncertain and the ‘right’ answers or path aren’t clear. Learn how to respond to difficult questions and how to use the advance care planning process to support your patients, teams, and help achieve care that is best for the child or young adult, in the right place, at the right time, by the right team.
