Chronic Disease Self-Management

Chronic Disease Self-Management is a program designed to help you gain self-confidence in your ability to control disease symptoms. This six week program will help you build skills to manage and cope with chronic conditions so that you can live a healthier life. This program was developed at Stanford University and complements professional services provided by your doctor or health car provider. This FREE program is sponsored by Arthritis Community Services. All participants will receive a reference book and a guided relaxation CD.
