Childhood and Adult Vaccine Clinic at Shaw Middle School

Vaccination event offering routine childhood vaccines and select adult vaccines. See the table below for available vaccines. This vaccination clinic is open to:

- Adults ages 19 and older who do not have insurance
- Children age 6-18 years, regardless of insurance coverage

No appointments. Walk-ins are welcome.

You will need to fill out a consent form for any individuals receiving vaccine at the event. To save time at the event, you may download, print and complete the demographic portion of the consent form for each individual receiving vaccine at this event and bring it with you. Please use the Child Consent Form for individuals who are under 19 years of age (0-18).
