Advanced Care Planning Session

What is ACP?
Advance care planning (ACP) is the process of discussing and documenting your healthcare preferences for situations where you may be seriously ill or unable to communicate. It helps ensure that your loved ones and healthcare providers understand your wishes, reducing stress and confusion during critical times.
