Aqua FIT with Lesley

Participant Ackowledgment

I, the participant, acknowledge that I am voluntarily participating in the Aqua FIT class offered by Lesley. I understand that participation in physical activities, including exercises in or around water, involves inherent risks that may result in injury.

I understand and acknowledge that:

Participation in Aqua FIT may involve strenuous physical activity and the natural risks associated with exercise in a water environment.

I am responsible for ensuring that I am physically capable of participating.

I assume full responsibility for any risks of injury or harm that may occur as a result of my participation.

I, the participant, release and discharge Lesley De Leon, the instructor, the residence, and any affiliates from any and all liability, claims, demands, or causes of action arising from participation in the class.

I understand that Lesley De Leon is not responsible for any injuries, accidents, or health conditions that may result from my participation.

By registering for the class, I agree to the terms of this waiver and confirm that I am voluntarily participating in the Aqua FIT class at my own risk.
