CVDACTION, developed by UCLPartners, is a translational data tool to support transformation of the CVD prevention pathway in primary care. CVDACTION runs on GP systems, with data visualised on Tableau or PowerBI dashboards. The dashboards identify and stratify patients with high-risk conditions whose care needs optimising, helping clinicians to prioritise and phase clinical activity over time.CVDAction@uclpartners.com
