SMNF is the managing entity of the city of Syracuse-owned Syracuse Community Connections located at 401 South Avenue in the city of Syracuse.Syracuse Model Neighborhood Facility, Inc. provides a network of human services throughout Onondaga County. The agency has particular experience in service provision to predominately African American, low-income residents. Through special events, outreach efforts, prevention, intervention, employment, and educational services, support, peer, and mentoring groups, and collaborative programs – people throughout the city and county interact with and benefit from agency services.Our MissionTo work with individuals, families, and communities to promote health and well-being through prevention, intervention, and education.
