You can download a form at LiveHealthyGC.com or pick one up from the Health Education & Outreach Unit at the Garrett County Health Department, Room 206

📒Log in the miles you walk/run each day. By the 5th of the following month, call (301-334-7730) or email (a2FpdGxpbiAhIGx1Y2FzIHwgbWFyeWxhbmQgISBnb3Y=) us your total miles for the month to be entered into a monthly drawing for prizes.

(Submit final form by November 7th to be eligible to win a grand prize!)🌟
