About this Event

Transition Planning After High School is a 1.5-hour primer class designed to help students with disabilities and the people who support them navigate life after high school. Participants will leave the class with a transition checklist as well as a broad overview of the services and support available to help them avoid the “transition cliff,” ensuring a smooth and successful transition into adulthood.

  • Overview of Home and Community Based disability services
  • Employment options after high school/transition school
  • Day support services after high school/transition school
  • How to access support for questions and next steps

Course Details

  • Thursday, September 11, 2025
  • 5:30 - 7:00 PM
  • In-person at Lifeworks Center for Disability Inclusion – New Hope, 7508 N 42nd Ave, New Hope, MN 55428
  • Ideal for students ages 14 to 22 – families are highly encouraged to attend
  • Free to attend – registration required
  • Each student participant may be accompanied by two support people (parent, guardian, PCA, etc.) for the class
  • Snacks and drinks provided
  • Quiet room available, fully wheelchair accessible, adult changing table in bathroom

Class Size is Limited

Only 15 student participants per session. Early registration is encouraged. A waitlist of 20 is available once the class fills. If there is a high interest via the waitlist, we will host this class again.

Meet Your Instructors

Keeri Tramm, Director of Disability Initiatives at Lifeworks, is an Autistic professional dedicated to fostering equity and empowerment for the disability community. With a career spanning nonprofit and for-profit sectors, she leverages her expertise in business and data analysis to drive impactful outcomes. Keeri holds a degree in Special Education and serves on the board of Minnesota NEAT, where she influences disability policy and practice. Her experience as a case manager in Oregon and Minnesota underscores her commitment to advocacy and creating lasting change. 

Carrie Grace (she/her) holds a master’s degree in human resource development and brings over a decade of experience in the social services field. She has worked across in-home and employment services and now specializes in helping people with disabilities navigate complex systems. As a Navigation Specialist and Certified Benefits Coach at Lifeworks, Carrie empowers individuals to take control of their futures with clarity and confidence.
