CFSS Open House

Join Circle of Life Home Care for a Community First Services and Supports (CFSS) Open House. This event is an opportunity to better understand the steps and meet in person with consultation service providers to make the transition from PCA to CFSS and continue receiving home care services in Minnesota.

• Ask questions and get support with next steps
• Light refreshments provided

This event is free of charge, open to all members of the community and is here to help navigate this mandatory program transition. We know that understanding the new CFSS transition process can be challenging. Let us help you complete it with ease.

The charter bus will wait approximately 10 minutes at each pick-up location with expected arrival at 12:30. Sign up for your spot when you register. Return transportation will also be provided, departing at 4pm.
