W12 Together, the NHS Primary Care Network and LBHF Family Champions are working together to hear from local residents about their experiences of GP services in North Hammersmith & Fulham.
We are inviting residents who live with a long-term or ongoing health condition to take part in a friendly, small focus group to share what is working well and where GP services could be improved. Your feedback will help shape how local primary care services are delivered in the future.
Residents living in North Hammersmith & Fulham who:
