Time: 9AM - 130PM or 5PM - 930PM

�CLINICAL SCHEDULE
Days: Saturday and Sunday
Time: 7AM-3:30PM

�TUITION: $1500.00

�PAYMENT PlAN
$375.00 due weekly Tuesday

�First payment due by class start date

�$100 state exam fee not included in tuition.

�Social Security Card
� QuantiFERON-TB Gold Test
( within previous 6 months )

�TO COMPLETE ENROLLMENT

�Allow 30-45 minutes to complete basic entrance exam.
