✅ TCMF Grant– Required Documentation Checklist
1. Annual Revenue- 2024 federal and state tax returns (filed and signed)
2. Business 10% Revenue DeclineProvide one of the following (showing at least 10% decline in 2023 or 2024):
- Form 1120 (pg. 1, Line 1c)
- Schedule C (Form 1040, pg. 1, Line 3)
- Form 990 (pg. 1, Line 12 – nonprofits)
3. Business Lease, Mortgage, or License- Signed lease, mortgage, or license (dated on or before Jan 1, 2022)
4. Hawaii Compliance Express Certificate of Vendor Compliance- Current & compliant certificate (within 60 days of application date)
(Not required for sole proprietors not registered with DCCA)
5. State of Hawaii General Excise Tax License(Only for sole proprietors not registered with DCCA)
- Current General Excise Tax License
6. IRS Tax Compliance Report (Letter 6201)(Only for sole proprietors not registered with DCCA)
- Report current within 60 days of application date
7. State of Hawaii Tax Clearance Certificate(Only for sole proprietors not registered with DCCA)
- Certificate current within 60 days of application date
8. Current Business OwnersProvide one of the following:
- State BB-1 Basic Business Application
- Articles of Incorporation
- Partnership Agreement
- Articles of Organization (LLC)
- Other approved ownership document
9. Proof of City Residency (Majority-Owned by City Residents)Provide two documents per owner (at least one from items 1–4):
- State Driver’s License
- Government-issued ID
- Voter Registration
- City real property tax bill (personal residence)
- Car registration
- Lease for personal residence
- Mortgage statement (personal residence)
- Utility bill (personal residence)
- Other approved proof of residence
