CONSENT FOR PARTICIPATION - HOLD HARMLESS AGREEMENT
I understand that by signing my child(ren) up, I certify that all the information below is correct and that the person(s) is physically fit to compete in golf. Also, by signing, I do hereby, for myself, executors, administrators, and person(s), waive release and forever discharge any and all rights and claims for damages which may occur or which may hereafter occur to the person(s) against the Columbus Recreation and Parks Department, The Greg Poston Golf Shop, volunteers, respective officers of the City of Columbus or The Greg Poston Golf Shop and/or assign for any and all damages which may be attained by the athlete(s) listed above.
CONSENT FOR USE OF INFORMATION AND PERSONA
I hereby grant to The Greg Poston Pro Shop the right to publish, broadcast, webcast, or disseminate in any other form or medium any or all of the following:
• Stories and/or information about the persona for use in news stories, publications, promotional materials, web features, and/or any other marketing purposes.
• Photographs, video, audio, and other images or likenesses of a minor for use in news stories, publications, promotional materials, web features and/or any other marketing purposes, promotional materials, web features, and/or any other marketing purposes.
• All photographs, video, audio, images, likenesses, stories, and other materials will remain the property of The Greg Poston Pro Shop.
