Sunlake Volunteer Fire Department Health Fair and Vaccine Clinic

The sign up sheet for the Health Fair/Vaccine Clinic is in the Billards Room. There are also consent forms and Vaccine Information Sheets. Please check with your Medicare or Insurance Carrier to assure they will pay for your vaccines.
When you come to the clinic please bring your consent form, an ID, and your Medicare or insurance card.
The Health Fair/ Vaccine Clinic is Wednesday, November 5, 2025 from 9am until 1 pm.
You must register by signing up on the sign up sheet or call Donna Schneider at 352-771-1052 or 712-574-0410.
Because of vaccine availability you must register or wait until the end of the clinic to receive your vaccines. Because of the number of people who had not registered last year and came early, many had to be turned away because we ran out of vaccine.
